You may fill out and print the following forms before visiting Lansdowne Foot & Ankle Center for the first time. FIlling out the forms requires the latest version of Adobe Acrobat Reader. If you do not have Acrobat Reader, you can download it for free.
- Patient Information Form (version for online completion)
- Patient Information Form (version for completion by hand)
- Financial Policy/HIPAA Form
- Notice of Privacy Practices. Hard copies of the Notice are available in the office.
